Chaotic. The agonized khyphon radiates a Chaotic aura.

Magic Resistance. The agonized khyphon has advantage on saving throws made against spells and other magical effects.

Sickness Carrier. When a creature hits the agonized khyphon with a melee attack using a weapon that does not have reach, it makes a DC 16 Constitution saving throw or becomes infected with demon fever. Whenever the creature attempts to take a long rest , it makes a DC 16 Wisdom saving throw or it gains no benefit from the rest. A protection from evil and good or magic circle spell cast on an infected creature prevents the nightmares for 24 hours, allowing it to benefit from a long rest. A heal spell or more powerful magic permanently cures demon fever. Alternatively, the agonized khyphon can choose to end the infection at any time.


ACTIONS

Multiattack. The agonized khyphon makes three attacks with its bandages.

Bandages. Melee Weapon Attack: +8 to hit, reach 15 ft., one target. Hit: 11 (2d6+4) force damage plus 7 (2d6) psychic damage and the target makes a DC 16 Charisma saving throw . On a failed save, the target is poisoned until the end of its next turn. If the saving throw fails by 5 or more, the target is poisoned and frightened until the end of its next turn.

Control Bandages (Recharge 6). Up to four bandages wrapped around the agonized khyphon’s body animate under its control. Alternatively, the agonized khyphon can choose to animate bandages it can see within 100 feet of it instead, provided that the bandages aren’t being worn or carried. Each animated bandage is an object with AC 20, 20 hit points, resistance to bludgeoning damage, and immunity to psychic and thunder damage. When the agonized khyphon uses multiattack on its turn, it can use each animated bandage to make one additional bandage attack. An animated bandage reverts to its inanimate state if reduced to 0 hit points or if the agonized khyphon is incapacitated or dies.


BONUS ACTIONS

Harm (Recharge 6). Melee Spell Attack: +9 to hit, reach 20 ft., one target. Hit: 60 necrotic damage and if the target is a creature it makes a DC 16 Charisma saving throw . On a failed save the creature becomes blinded, deafened, poisoned, or afflicted with a disease of tier 2 or lower (chosen by the agonized khyphon). The creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.


REACTIONS

Quick Reaction. The agonized khyphon has 2 reactions each round. Only one reaction may be used to make an opportunity attack .

Deflect Attack. The agonized khyphon strikes a melee, ranged, or spell attack that would hit it, reducing the attack’s damage by 18 (4d6+4). When deflecting a spell attack with an extra effect, the agonized khyphon makes an attack roll opposed by a spellcasting ability check by the spellcaster. On a success, the spell effect is negated.

Description

Bandages writhe beneath and around this human-shaped giant, the decrepit straps wet with pus leaking out of the countless eyes peeking out from beneath. There is a golden glow coming off of its bandages, but the khyphon is ensconced within disturbing and malevolent magenta as well, a color that inspires suffering looked at directly.

The fall of a khyphon is always tragic—many temple sermons preach that the greatest healers often fail to spot their own symptoms, using these broken celestials as prime examples. The relentless horror and misery they have witnessed have turned mercy angels into living hosts for the very same suffering they were tasked to destroy. What is worse is that agonized khyphonim are unaware there is anything wrong, still continuing their work like a putrid rag trying to clean an open sore.
